Responsibilities
- Agissant comme personne-ressource auprès de l’équipe interprofessionnelle
- Assurant le suivi des résultats anormaux et la coordination des démarches nécessaires
- Surveillant les tests prescrits en GMF-R et les demandes à l’accueil clinique
- Offrant enseignement, soutien à l’autogestion et suivis requis aux patients
- Orientant les patients vers les services appropriés et assurant la liaison avec les partenaires externes
- Participant au développement d’outils cliniques et au suivi de la qualité des soins
- Renforçant les liens de collaboration avec les partenaires du réseau
Requirements
- DEC en soins infirmiers (obligatoire)
- 3 ans d’expérience en soins infirmiers (souhaité)
- Intérêt marqué pour la relation patient et la collaboration interdisciplinaire
