About the role

  • Clinical Supervisor overseeing PSW and HSW teams for ParaMed, Canada's home healthcare provider. Assessing care quality, resolving clinical issues, and supporting safe community-based patient services.

Responsibilities

  • Supervise and support PSWs and HSWs to ensure compliance with legislated standards, company policies, and procedures
  • Respond to and resolve concerns from community staff, patients, families, and team members
  • Provide leadership through regular huddles, face-to-face meetings, and one-on-ones
  • Initiate patient safety incident investigations and documentation, ensuring timelines and action plans are met
  • Collaborate with nursing team members on clinical documentation and funder reporting requirements
  • Engage patients in collaborative problem solving, health teaching, rapport building, and care-needs assessment
  • Engage staff in problem solving through education, coaching, and mentoring
  • Maintain current knowledge, skills, and attitudes to provide safe, efficient, and effective clinical service

Requirements

  • Current RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions
  • Minimum of 2 to 5 years’ experience as a practicing Nurse
  • Comprehensive patient health assessment skills are required
  • Understanding of the principles of community nursing practice
  • Proven experience leading and managing a team
  • Knowledge of clinical documentation standards and practice guidelines is required
  • Excellent organizational, interpersonal, and teaching skills
  • Commitment to professional development in clinical practice relevant to home health care
  • Effective critical thinking and problem-solving skills
  • Must possess a valid driver’s license and access to reliable transportation
  • Able to provide a current, clear Vulnerable Sector Screening (VSS)
  • Must be able to work in Oshawa or Scarborough, Ontario
