About the role

  • Registered Nurse providing scheduled community nursing visits across Kenora, Keewatin, and Sioux Narrows, Ontario. Assessing needs, managing care plans, and supporting patients at home.

Responsibilities

  • Provide scheduled nursing visits to patients in various settings
  • Care for patients of all ages with differing complexity of care needs
  • Help patients remain comfortable in their homes for as long as possible
  • Assess patients’ individualized needs using appropriate assessment tools and techniques
  • Execute and continuously monitor care plans
  • Engage patients collaboratively by building rapport and trust
  • Maintain and update professional patient records related to patient encounters and plans

Requirements

  • Registration with the CNO as a Registered Nurse in good standing
  • Valid driver’s license and access to a vehicle is required
  • Experience in delivering community care is an asset
  • Reliability and flexibility
  • Willingness to travel to and from patient residences within a designated area
  • Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
