About the role

Registered Nurse providing in-person and outreach nursing care to marginalized individuals. Supporting comprehensive primary care through collaboration with interprofessional team members in London, ON.

Responsibilities

Provides front-line outreach as well as onsite/in-person nursing care to people facing barriers to accessing health care
Facilitates connections to internal and external referrals
Works to full nursing scope to support marginalized individuals significantly affected by poverty, substance use, mental health, and housing instability
Connects with broader integrated resources of the TVFHT and LIHC

Postsecondary diploma or degree (BScN) from a recognized college or university
Current registration with the College of Nurses of Ontario
Minimum 2 years of nursing experience in a community setting or a combination of community and hospital/public health
Awareness and understanding of the social determinants of health and health promotion
Ability to practice from social justice, trauma and violence informed care, and harm reduction frameworks
Clinical knowledge of mental health care, substance use, concurrent disorders, dual diagnoses, and application of current best practices, assessments and interventions
Ability to work with people of diverse backgrounds with an inclusive and anti-oppressive lens
Excellent written and oral communication/interpersonal skills
Strong computer skills including the use of Office 365 and Electronic Medical Records (Accuro currently in use)
Ability to travel to multiple sites during the work day
