Practice Improvement Coach advancing primary care quality for Doctors of BC. Facilitating quality improvement, coaching, change management and practice support across British Columbia.
Responsibilities
Support improvements in primary care in family practice settings
Develop quality improvement capacity across practice teams through tailored practice facilitation
Enable doctors to adopt and sustain the attributes of the Patient Medical Home
Work as part of a support team including Health Technology Coaches, Panel Management Coaches, peer mentors and program staff
Collaborate with Divisions of Family Practice, health authorities, the Ministry of Health and other community partners
Deliver practice support services aligned with FPSC direction and practice and community needs
Build common understanding of issues and priorities between client groups
Establish shared objectives and obtain feedback and assistance
Lead planning and implementation of change programs in a multi-stakeholder environment
Identify process enhancement opportunities and recommend strategic changes
Establish monitoring and feedback expectations and review performance trends
Lead pilots and planned functional change initiatives
Analyze issues, performance, work errors, successes and failures to develop improvement strategies
Develop short-term program strategies aligned with organizational priorities
Partner with client groups and negotiate effectively with diverse stakeholders
Facilitate small group sessions and effective meetings
Travel within the region and to its facilities and communities
Requirements
Bachelor's degree in Health Services, Health Information Management, Business Administration, or related field and 3-5 years of experience, or an equivalent combination of education and experience
Solid understanding of the BC health system and primary care operational context
Experience working directly in a family practice or supporting role is an asset
Understanding and application of quality improvement methodologies, ideally in health care settings
Ability to connect workflow, structured data capture and meaningful reports
Ability to extract and use measures, evaluate success, and motivate providers and teams
Experience applying coaching approaches using questions and constructive feedback
Familiarity with one or more EMR systems, including registries, scheduling, visit flow and registry-based recalls
Understanding of the BC health care system, primary care operational models, financial incentives, patient conditions and journeys, processes, team roles and structures
Experience and training in change management principles
Ability to assess change readiness and build commitment using coaching techniques
Skilled at facilitating small group sessions and preparing effective meetings
Ability to use methods and questioning skills to maximize group participation
Ability to work effectively within and across teams and develop trusting working relationships
Passion for improving primary care and supporting physicians
Service orientation and ability to anticipate and exceed customer expectations within a defined scope
Ability to navigate diverse personalities and social circumstances and adapt communication styles
Must travel within the region and to facilities and communities
Must have access to reliable transportation

Salary range of $70,856 CAD to $88,570 CAD
