About the role

  • School Nurse/Community Visiting Nurse providing care in both school and home settings. Working with patients of various ages and adapting care to their needs with a supportive team.

Responsibilities

  • Care for patients in both school and home settings
  • Work with patients within pediatric ages with differing complexity of care needs
  • During community visits work with patients of all ages with differing complexity of care needs
  • Help patients remain comfortable in their homes for as long as possible
  • Using appropriate assessment tools/techniques to assess patients’ individualized needs
  • Executing and continuously monitor care plans
  • Engaging patients using a collaborative approach by building rapport and trust
  • Maintaining and update professional patient records related to patient encounters and plans

Requirements

  • Registration with the CNO as a Registered Practical Nurse in good standing
  • Valid driver’s license and access to a vehicle is required
  • Experience in Pediatric care is an asset
  • Experience in delivering community care is an asset
  • Reliability and flexibility
  • Willingness to travel to and from patient residences within a designated area
  • Passion for making a difference in your community
  • Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
