About the role

  • Registered Dietitian/Diabetes Educator providing diabetes care management and group education at WELL Health Diagnostic Centres. Supporting insulin therapy, glucose monitoring, and individualized chronic disease plans.

Responsibilities

  • Provide individual and group diabetes education sessions to patients
  • Assess patients’ diabetes knowledge and readiness, adapting education and care plans to individual needs
  • Teach insulin administration, titration, management, and self-monitoring of blood glucose according to medical directives
  • Collaborate with healthcare teams and patients to develop and implement comprehensive diabetes management plans
  • Incorporate nutrition, activity, and medication into individualized care plans
  • Use blood glucose meters, continuous glucose meters, and insulin pump therapy technologies
  • Monitor glucose records and treatment effectiveness, adjusting plans to support optimal blood glucose control
  • Apply behaviour change counselling techniques and support patients with diabetes-related challenges
  • Assess readiness for and manage patients on insulin pump therapy
  • Maintain current knowledge of diabetes research, treatments, and developments, including CDE qualifications
  • Complete clear chart notes in the Accuro clinic EMR
  • Develop patient handouts and provide consultations to community groups, partners, and team members
  • Mentor nutrition volunteers, dietetic interns, and community CDEs
  • Present at professional seminars, conferences, and lectures
  • Maintain clinic medication samples and non-medicinal supplies
  • Foster relationships with external contacts and perform other assigned duties

Requirements

  • Bachelor of Human Ecology, Foods, and Nutrition Major (or equivalent) and completed an accredited Dietetic Internship, or Bachelor of Nursing
  • Valid RD license in good standing with the College of Dietitians of Alberta, or valid RN license in good standing with CARNA
  • Clinical competence in an expanded dietetic or nursing role
