About the role

  • Nurse Practitioner Team Lead providing in-person and telemedicine primary care for substance use, mental health and gambling concerns. Supporting clinical leadership, education and service improvement at The Jean Tweed Centre.

Responsibilities

  • Provide non-urgent primary care assessments in person or by telemedicine
  • Provide direct clinical assessments, consultations and primary care within the RN extended class scope of practice
  • Determine clients’ needs for urgent or emergent care
  • Provide telemedicine assessments to determine the need for emergency room visits or other care
  • Establish caring and therapeutic relationships with patients
  • Provide individual and group education on health topics, including medications for cravings and withdrawal
  • Collaborate with interprofessional teams on care planning and follow-up for high-risk or complex health problems
  • Assist with ethical and difficult situations using communication and conflict management skills
  • Collect and analyze data and prepare reports on program indicators
  • Monitor telemedicine services and identify issues requiring management attention
  • Recommend service improvements to enhance client and staff safety and quality of care
  • Support strategic plans, objectives, programs and services in collaboration with the Executive Director and Clinical Director
  • Support development and updating of clinical and IPAC policies
  • Work with community and hospital partners to strengthen programs and advance services
  • Maintain relationships with external networks, professional organizations and service providers
  • Support team learning and professional development
  • Follow harm reduction, trauma-informed, women-centred and anti-oppressive practices
  • Adhere to occupational health and safety requirements and organizational policies
  • Promote client service, collaboration, accountability, inclusion, equity and continuous learning

Requirements

  • Master’s Degree in Nursing
  • Current registration and in good standing with the Colleges of Nurses of Ontario
  • Nurse Practitioner Certification from a recognized program
  • Membership with the Nurse Practitioner’s Association of Ontario (NPAO)
  • Minimum of 2-4 years of clinical experience in a hospital or community health setting, including 2 years caring for clients and families with substance use, mental health and gambling concerns
