About the role
- Nurse Practitioner delivering acute, chronic, and palliative resident care at AgeCare, which operates supportive long-term care communities. Providing clinical leadership and resident-focused care planning.
Responsibilities
- Work collaboratively on the delivery of resident care to meet individual needs within the level of competency
- Provide quality and safe resident care aligned with resident needs
- Collaborate and liaise with the MD, DOC, and interdisciplinary team in care delivery
- Assess, diagnose, and evaluate acute and chronic illness and changes in residents’ conditions
- Prescribe treatments, diagnostics, and follow-up care
- Support and advocate for residents’ best interests while engaging residents, families, and substitute decision makers regarding care
- Participate in resident care conferences
- Provide clinical leadership to long-term care team members regarding resident move-in/move-out, hospital discharge planning, palliative and end-of-life care, pain management, dementia care, and responsive behavior management
- Identify, analyze, and interpret trends in resident care and professional nursing practice
- Determine priorities for educational programming for team members, residents, and families
Requirements
- Minimum one year clinical and/or working experience in geriatric, rehabilitation, or long-term Care nursing
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice
- Some leadership knowledge/experience is considered an asset
- Certifications obtained or in progress that include gerontology, mental health, behavioral support, and palliative care are an asset
- Working knowledge of the Long Term Care Act, Program Standards for Long Term Care, Health Disciplines Act, Occupational Health & Safety Act, Pharmacy Act, Food and Drugs Act (Canada), Narcotic Control Act (Canada) and Coroners Act is considered an asset
