• Work collaboratively on the delivery of resident care to meet individual needs within the level of competency
• Provide quality and safe resident care aligned with resident needs
• Collaborate and liaise with the MD, DOC, and interdisciplinary team in care delivery
• Assess, diagnose, and evaluate acute and chronic illness and changes in residents’ conditions
• Prescribe treatments, diagnostics, and follow-up care
• Support and advocate for residents’ best interests while engaging residents, families, and substitute decision makers regarding care
• Participate in resident care conferences
• Provide clinical leadership to long-term care team members regarding resident move-in/move-out, hospital discharge planning, palliative and end-of-life care, pain management, dementia care, and responsive behavior management
