About the role

  • Patient Care Coordinator supporting Calgary primary care physicians and teams through EMR-based patient coordination. Managing patient outreach, referrals, scheduling, and panel maintenance for a non-profit primary care network.

Responsibilities

  • Support routine panel maintenance and patient validation processes for ongoing panel management
  • Confirm primary medical doctor, demographics, and patient information while performing patient outreach
  • Identify missing vitals, lab results, screenings, and referrals to external organizations
  • Ensure recent test and procedure results are available to providers through Alberta Netcare
  • Create and maintain accurate project notes, including meeting minutes and ad hoc patient list requests
  • Contact patients by telephone, email, and text for project-related outreach
  • Support clinicians with scheduling using EMR systems and digital health platforms
  • Coordinate patient care with appropriate clinicians, including referral processing
  • Ensure patients receive required appointment information and pre-visit instructions
  • Coordinate referrals to the medical neighbourhood for care outside the Patient’s Medical Home
  • Support physician members and team members to improve overall patient outcomes

Requirements

  • Medical Office Assistant (MOA) certificate or diploma, or post-secondary diploma or certificate in a related area such as Licensed Practical Nurse, Medical Secretary, or Unit Clerk
  • Minimum of two years of required experience working within an interprofessional care team model
  • Minimum of two years of required experience with medical records management
  • Minimum of two years of required experience maintaining filing systems and basic databases
  • Microsoft Office: Word, Excel, Outlook, PowerPoint, Teams
  • Virtual patient care platforms
  • Electronic Medical Records (EMR) systems
  • Clear Criminal Background check required to start
  • Must be located within Calgary or the surrounding area
