About the role

  • Care Navigator guiding older adults and caregivers through Sailor Health’s mental health intake process. Coordinating therapist matching and compassionate patient support remotely from Canada.

Responsibilities

  • Serve as the first point of contact for patients and their families reaching out to Sailor Health
  • Guide older adults and caregivers through the intake process
  • Provide patient support, care coordination, and emotional guidance
  • Help patients navigate into care, including matching them with the right therapist
  • Support patients throughout the intake and care journey
  • Help improve care access for older adults and their families
  • Take initiative and help shape key processes on a small, agile team

Requirements

  • Must be based in Canada
  • Empathetic and clear communicator
  • Highly organized and detail-oriented
  • Comfortable learning Google Workspace, EHR platforms such as Healthie, and CRMs such as HubSpot
  • Experience in healthcare, customer service, or administrative support is welcome but not required
  • Interest in mental health and healthcare access
  • Comfortable working Eastern Time Zone hours
  • Willingness to work one of the listed 9a–6p ET or 12p–9p ET shifts
