Bilingual RN case manager supporting specialty-therapy patients through McKesson’s healthcare Patient Support Program. Coordinating education, treatment access, reimbursement, and clinic care across the GTA.
Responsibilities
Serve as the primary clinical contact for patients, physicians, and clinical staff enrolled in the Patient Support Program
Support patient onboarding and facilitate timely access to prescribed specialty medications
Educate patients about their conditions, treatment expectations, medication administration, side-effect management, and treatment adherence strategies
Provide injection and self-administration training in the clinic, virtually, or in the patient’s home
Collaborate with healthcare professionals to coordinate care and communicate relevant updates
Support reimbursement and treatment-access activities, including collecting required documentation and assisting with patient enrollment processes
Review patient files and gather information required for reimbursement activities or clinical programs
Monitor patient progress and promote treatment adherence
Identify situations requiring additional clinical or operational support and escalate them to the appropriate parties
Build and maintain trusted relationships with physicians, clinical staff, pharmacies, and patients
Accurately document patient interactions and program activities
Contribute to program initiatives and continuous improvement activities
Requirements
Hold an active, unrestricted Registered Nurse (RN) license in Ontario
Be fluent in French and English, both verbally and in writing
Have previous experience working in a Patient Support Program (PSP)
Hold a valid Ontario driver’s license and have access to a reliable vehicle
Be able and willing to travel up to 60% of the time (approximately three days per week) throughout the Greater Toronto Area
Demonstrate strong patient education and communication skills
Be proficient in Microsoft Office and electronic documentation systems
Additional active nursing licenses in other Canadian provinces (preferred)
Experience supporting specialty pharmacy, biologic, immunology, rheumatology, dermatology, gastroenterology, or other specialty therapeutic areas (preferred)
Experience providing injection training and patient education (preferred)
Knowledge of reimbursement, patient access, or specialty medication programs (preferred)
Experience working directly with physicians’ offices, infusion clinics, or specialty clinics (preferred)
Familiarity with biologic therapies and pre-treatment requirements, including laboratory testing, diagnostic imaging, and screening protocols (preferred)
Strong relationship-building and stakeholder management skills (preferred)
Experience working independently in a field-based healthcare role (preferred)
