About the role

  • Field Nurse Case Manager coordinating patient support programs for Sentrex Health Solutions. Supporting patients, physicians, funding access, therapy adherence, and clinical case management across New Brunswick.

Responsibilities

  • Adhere to Sentrex Policies and Procedures
  • Perform HCP and patient registration activities according to program protocols
  • Process pertinent data-entry activities
  • Respond to telephone and web-based inquiries about program services, disease states, and products
  • Provide medical support and education to patients and health care providers
  • Complete specialized training for specific therapies
  • Provide inbound and outbound telephone support, including maintenance and adherence calls
  • Manage cases through clinical monitoring, data collection, follow-up, compliance, therapy disruption/discontinuation monitoring, adverse-event reporting, and liaison with primary health care providers
  • Work with care navigators and physicians to secure therapy funding and conduct alternative funding searches
  • Attend patient meetings and conferences as needed
  • Identify and report adverse events to Health Canada, Med Info, and relevant source and third-party systems
  • Maintain confidentiality of client and corporate information
  • Liaise with physicians’ offices, pharmacies, infusion service providers, and other parties to coordinate patient and physician services
  • Participate in continuing education activities
  • Enroll patients and complete required follow-ups
  • Support and advocate for patients and HCPs throughout enrollment, coverage renewals, reimbursement, and the patient journey
  • Schedule appropriate next steps to ensure continuity of service delivery
  • Travel to physician offices as requested
  • Perform additional duties assigned by the manager

Requirements

  • Must be a Registered Nurse in good standing with the College of Nurses of their respective province
  • 2-4 years of clinical experience
  • Experience in Patient Support Program is an strong asset
  • Experience in oncology, gastro or rheumatology
  • Proven ability to work in a team environment
  • Ability to resolve conflict in a professional matter
  • Bilingual in English and French is required
  • Strong analytical and problem-solving skills
  • Strong customer service skills
  • Exceptional organizational skills
  • Willingness and ability to travel up to 75% within the assigned geographic region
  • Availability to work Monday to Friday within 8:00am-8:00pm EST
