About the role
- Physiotherapist delivering in-home and virtual rehabilitation for The Care Company’s community home care clients. Managing assessments, treatment plans, mobility goals, and interdisciplinary care.
Responsibilities
- Complete comprehensive in-home physiotherapy assessments within required referral timeframes
- Perform mobility, balance, gait, falls risk, range of motion, strength, pain, and functional mobility assessments
- Develop individualized, evidence-based treatment plans
- Provide follow-up visits involving therapeutic exercise, gait training, balance retraining, mobility progression, pain management, post-operative, neurological, and cardiorespiratory rehabilitation, and falls prevention
- Develop home exercise programs and educate clients and caregivers
- Recommend and train clients in the safe use of mobility aids, gait aids, and assistive devices
- Collaborate with Occupational Therapists on equipment and home safety recommendations
- Support clients and families through education, coaching, and practical strategies
- Collaborate with Occupational Therapists, nurses, Personal Support Workers, physicians, care coordinators, case managers, and Ontario Health atHome
- Monitor outcomes and modify treatment plans
- Maintain accurate, timely, clinically sound documentation
- Communicate with the office regarding caseload availability, scheduling changes, urgent referrals, and client progress
- Independently manage caseload and schedule with administrative, scheduling, clinical, and technical support
- Contribute to service development, RFPs, quality improvement initiatives, innovative care models, clinical resources, and best practices
- Mentor Physiotherapy students and support onboarding and mentorship of future Physiotherapists
Requirements
- Registered and in good standing with the College of Physiotherapists of Ontario (CPO)
- Experience providing home and community care, particularly with HCCSS and/or Ontario Health atHome clients, is strongly preferred
- Community physiotherapy and/or hospital discharge, rehabilitation, or transitional care experience will be considered
- Demonstrated skill in mobility assessments, falls prevention, gait training, balance retraining, therapeutic exercise prescription, neurological rehabilitation, orthopedic rehabilitation, and post-operative rehabilitation
- Experience prescribing mobility aids and collaborating with interdisciplinary teams
- Strong understanding of community discharge planning, mobility supports, caregiver education, and promoting aging in place
- Strong electronic documentation skills
- Experience coordinating with interdisciplinary teams and communicating with physicians and case managers
- Ability to work independently in the community and produce clear, timely, clinically appropriate documentation
- Valid driver's licence and access to reliable transportation, or ability to travel efficiently within the designated service area
