Aftercare Counsellor supporting women transitioning from intensive substance-use treatment to community life at Jean Tweed Centre. Facilitating recovery groups, follow-up care, referrals, and outcome tracking.
Responsibilities
Provide and launch evidence-informed aftercare groups, including SMART Recovery, DBT, and MBRP, for clients completing an intensive program.
Provide follow-up support for up to six months post-discharge through calls and check-ins.
Provide referrals and follow up with institutions to promote seamless integrated care.
Identify and facilitate access to housing, financial aid, family support, crisis intervention/prevention/support, and advocacy resources.
Build linkages with agencies, supporting organizations, and social service systems.
Maintain documentation of client interactions, interventions, and collateral contacts.
Monitor client progress and prepare reports and letters.
Develop and maintain attendance tracking and evaluation tools.
Coordinate care and referrals with internal teams and external service providers.
Participate in case conferences, supervision, professional development, staff development, and personal learning.
Collaborate with the supervisor on performance discussions and professional goals.
Conduct responsibilities using harm reduction, trauma-informed, women-centred, and anti-oppressive practices.
Follow occupational health and safety requirements and Jean Tweed Centre policies and procedures.
Perform other duties supporting organizational goals.
Maintain client service orientation and work collaboratively with accountability, inclusion, equity, diversity, and continuous learning.
Requirements
Master’s degree in Social Work, Psychology, Counselling, or related field.
Registration in good standing with relevant regulatory body (e.g., Ontario College of Social Workers and Social Service Workers, College of Registered Psychotherapists of Ontario).
3 years of experience in the field of substance use, mental health and trauma-informed practice.
Experience working with women who have been involved in the shelter system, criminal justice system, or had difficulty accessing appropriate health services.
Group facilitation and prgram development skills.
Ability to work collaboratively within multidisciplinary teams.
Strong computer skills including working knowledge of Microsoft Office.
Knowledge of community resources and supports in the Toronto area.
Ability to prepare and present information in both formal and informal settings.
First Aid/CPR certification.
Canadians and Permanent Residents will be given priority.
