About the role
- Clinical intake specialist connecting addiction and mental-health clients with ADAPT community services. Assessing needs, coordinating referrals, crisis support, and clinical intake documentation.
Responsibilities
- Provide information, support, and guidance to individuals seeking help for addiction-related problems
- Connect clients to community resources and match people with appropriate services
- Establish rapport with clients and conduct assessments
- Review confidentiality as needed
- Provide informed internal and external referrals
- Engage in follow-up as needed
- Provide advocacy and crisis intervention when required
- Assess and evaluate needs related to addiction concerns
- Provide basic counselling, reassurance, support, and validation to distressed callers
- Conduct interviews and schedule clinical intake appointments
- Gather information about addiction history, substance use patterns, mental health status, and co-occurring conditions for case manager triage
- Provide advocacy and short-term crisis stabilization
- Conduct risk assessments and safety planning
- Communicate with clients and ADAPT counsellors/case managers to support appropriate interventions
- Resolve enquiries about ADAPT information, client information and referrals, and community resources
- Provide information about community health and social service agencies and connect callers to other agencies when appropriate
- Review OneLINK and self-referrals for completeness, initial suitability, and risk using standardized eligibility criteria
- Triage referrals according to urgency, clinician appointments, or community resources such as crisis services
- Collect, track, and report statistical service data
- Track calls and voicemails in the administration log sheet
- Complete screening and clinical intake documentation and enter required information into Outlook and/or EMHWare
Requirements
- University degree/diploma in Social Work, Social Service and/or Health-related field plus Addiction Studies certification, or equivalent combination of experience and education
- Current CPR and First Aid certification is required
- ASIST training or equivalent is required
- CPI training or equivalent is required
- 2 years of related experience, preferably in health care, addictions and mental health, social services, or community services
- Knowledge of addictions services and programs offered by ADAPT and externally in the community
- Intermediate knowledge of addictions/mental health treatments and explanatory theories
- Intermediate knowledge of substances and pharmacological knowledge to determine client needs
- Interviewing skills to identify and respond to callers’ unique needs
- Ability to deliver culturally responsive services
- Cultural knowledge of key populations in Halton Region
- Ability to interact effectively with people across cultures, abilities, genders, ethnicities, and races
- Knowledge of discrimination history in America, particularly race and racism, and disparities affecting marginalized communities
- Excellent clear, thorough, accurate, and timely documentation skills
- Knowledge of French is an asset
- Information and Referral Specialist Certificate is an asset or willingness to obtain it
- Registered Social Service Worker or certification with the Canadian Addiction Counsellors Certification Federation is an asset
