Clinical Coordinator supporting French-language mental-health stepped care at a Winnipeg community health centre. Coordinating assessments, referrals, crisis support, partnerships, and program evaluation.
Responsibilities
Respond to inquiries, coordinate appropriate services, and conduct in-person or virtual clinical assessments to develop service plans
Provide empathetic support and crisis intervention
Coordinate communication among service providers and formal and informal supports
Build referral networks and maintain resource directories
Strengthen relationships with community and program partners
Support program implementation, outreach, promotion, awareness activities, and community events
Contribute to evaluation tools and processes
Collect, document, and report program data and outcomes
Obtain consent and safeguard personal information in accordance with PHIA, FIPPA, and professional standards
Communicate concerns promptly and represent the Centre professionally
Comply with organizational, safety, legislative, regulatory, and accreditation requirements
Requirements
A BSW, OT, PT, BSc PN, RPN, BN, SLP, RD, or related health or human services degree
Membership in good standing in the applicable professional association or regulatory body
Proficiency in both of Canada’s official languages, orally and in writing
Experience in mental health or clinical coordination
Knowledge of Indigenous history and its impacts
Commitment to psychosocial rehabilitation and recovery, primary health care, trauma-informed practice, and person-centred planning
Excellent communication, conflict-resolution, relationship-building, critical-thinking, and organizational skills
Ability to support vulnerable populations, manage competing priorities, and work effectively in a fast-paced environment
Proficiency in Outlook and Word
Ability to perform the physical duties of the position in varied weather and environmental conditions
Subject to a Criminal Record Check including Vulnerable Sector Search, Adult Abuse Registry Check, and Child Abuse Registry Check; successful candidate responsible for service charges
