Addictions counsellor providing counselling, crisis intervention, and client navigation for ADAPT, an Ontario addiction and mental health agency. Coordinating warm transfers and wellness plans across Halton Region.
Responsibilities
Provide group and individual counselling
Facilitate active transfer and navigation of clients to subsequent needed services across the region
Conduct addictions and mental health assessments
Deliver acute stabilizing interventions
Evaluate interventions and treatment
Facilitate development and implementation of clients’ personalized wellness plans
Provide one-to-one instrumental support based on client needs
Develop, deliver, and evaluate group counselling
Provide crisis support
Engage, support, and educate families
Maintain health records to a high standard of quality
Conduct community engagement
Engage interprofessionally and inter-agency; build relationships and collaborate
Identify next-stage services and facilitate navigation and warm transfers
Complete statistical information according to Ministry and Program requirements
Participate in continuous quality improvement cycles
Requirements
University degree/diploma in Social Work, Social Service, and/or a health-related field plus Addiction Studies certification
Current CPR and First Aid certification required
ASIST training or equivalent required
CPI training or equivalent required
At least 5 years of relevant experience
Experience with individual, family, and group counselling
Experience in community development and networking
Experience in record keeping and file management
Advanced life skills counselling, supportive counselling, crisis intervention, advocacy, and group work
Ability to manage clients with complex needs in varying stages of change
Strong communication and active listening
Ability to present complex clinical information in writing and verbally
Ability to read and understand operating instructions, procedure manuals, and complex clinical reports
Broad knowledge of addiction treatments and explanatory theories, mental illness, and community resources
Ability to function independently, maintain effective working relationships, work under pressure or deadlines, work outside regular office hours as required, and use sound judgment
Attention to detail and rapport-building interpersonal skills
Awareness of ethical boundaries and clinical reporting obligations
Ability to deliver culturally responsive services
Cultural knowledge of key populations in Halton Region
Ability to communicate and interact effectively across cultures, abilities, genders, ethnicities, and races
Knowledge of the history of discrimination in America, particularly race and racism, and resulting disparities affecting marginalized communities
Active time management and scheduling
Proficiency with computer programs and electronic databases
Ability to fulfill duties and work hours from a remote/home location when required
Professional registration with the Ontario College of Social Workers and Social Service Workers is an asset
Knowledge of French is an asset
