About the role

  • Transitional Aged Youth Case Manager delivering clinical services and case management support for youth addiction. Collaborating with various service providers to enhance service integration.

Responsibilities

  • Deliver clinical services to Transitional Aged Youth (TAY), ages 16 – 24, by providing clinical assessments and developing and implementing clinically sound treatment plans
  • Provide intensive case management support in an outreach model of service
  • Work directly/ cooperatively with the ADAPT TAY Coordinator/ Youth Addictions Manager and team as well as with TAY Project Partners and a wide range of service providers, to increase service integration for transitional aged youth
  • Provides specialized assessments to Transitional Aged Youth, or to collateral clients associated with the youth, through the use of diagnostic tools and structured interviews
  • In consultation with the Client, develops an individual Treatment Plan for the Client
  • Provides individual, couples, family and/or group counselling to address the Clients’ needs
  • Refers Clients to other appropriate support services and agencies according to the Clients’ needs, with specific attention to supporting referrals between youth and adult mental health and addictions services

Requirements

  • University degree/ diploma in Social Work and/ or Health-related field plus Addiction/ Concurrent Disorders studies certification, or equivalent combination of experience and education
  • Registered Social Service Worker is an asset; Current CPR and First Aid certification is required
  • ASIST training or equivalent is required. CPI training is an asset. CACC certification is an asset
  • At least 2 years working experience with youth and young adults in the area of addictions or related field
  • Experience in community development and networking; Record keeping and file management; Basic life skills counselling, supportive counselling, crisis intervention, advocacy, and group work; Applied knowledge of addictions treatments and explanatory theories, mental illness, and community resources
  • Managing clients with complex needs in varying stages of change
