About the role
- Program Facilitator responsible for conducting group sessions and assessments in ADAPT's Back on Track program. Collaborating with participants and community agencies while promoting recovery and support.
Responsibilities
- Provides group facilitation and counselling services to participants of the ADAPT BOT program
- Conducts Back on Track group sessions as required
- Registers participants for all appropriate Back on Track services
- Conducts Back on Track assessments
- Assigns participants to either education group or treatment group according to assessments
- Provides consultative services to community agencies and maintains accurate participant records
- Completes Serious Incident Reports and submits “medical review” information
- Participates in training sessions and provides input into operational planning
Requirements
- University degree/ diploma in Social Work/ Social Service and/ or Health related field plus Addiction Studies certification, or equivalent combination of experience and education
- Must complete a security clearance, as required by the Ministry of Transportation-Ontario
- 3 years of counselling experience in addictions or a related field
- Experience facilitating online learning
- Non-violent crisis intervention, First Aid, CPR and ASIST certifications are assets
