About the role

  • Program Facilitator responsible for conducting group sessions and assessments in ADAPT's Back on Track program. Collaborating with participants and community agencies while promoting recovery and support.

Responsibilities

  • Provides group facilitation and counselling services to participants of the ADAPT BOT program
  • Conducts Back on Track group sessions as required
  • Registers participants for all appropriate Back on Track services
  • Conducts Back on Track assessments
  • Assigns participants to either education group or treatment group according to assessments
  • Provides consultative services to community agencies and maintains accurate participant records
  • Completes Serious Incident Reports and submits “medical review” information
  • Participates in training sessions and provides input into operational planning

Requirements

  • University degree/ diploma in Social Work/ Social Service and/ or Health related field plus Addiction Studies certification, or equivalent combination of experience and education
  • Must complete a security clearance, as required by the Ministry of Transportation-Ontario
  • 3 years of counselling experience in addictions or a related field
  • Experience facilitating online learning
  • Non-violent crisis intervention, First Aid, CPR and ASIST certifications are assets
