About the role
- Social Worker supporting admissions, resident care, and family services at Extendicare’s Tecumseh long-term care home. Coordinating multidisciplinary social work programs for seniors and caregivers.
Responsibilities
- Receive and respond appropriately to inquiries pertaining to Extendicare Inc. relating to placement, lifestyle, policies and procedures
- Conduct tours of the home with applicants, prospective applicants and/or their advocates
- Carry out pre-admission procedures, including screening applications, applicant correspondence, pre-admission visits and assessments, and communication with placement agencies
- Coordinate new admissions and accompany new Residents and their families/advocates through the admission process
- Promote and facilitate a Resident-oriented environment and act as a Resident/family advocate
- Plan, organize and deliver social work programs/services addressing Residents' and families'/advocates' psychological needs
- Participate in multidisciplinary care planning and program/service development
- Facilitate the development and/or functioning of positive Resident groups
- Organize and participate in Resident Care Conferences and family conferences
- Organize regular Family Orientation programs
- Provide educational and consultative services on social work-related issues through staff orientation, staff development, and educational seminars
- Complete documentation and statistical reports for quality management, departmental evaluation, trend identification, policy setting, and future program planning
- Uphold Extendicare's Code of Conduct and complete other assigned duties
Requirements
- Must be registered as a Social Worker under the Health Professions Act, 2000
- Graduate of a recognized school of social work with a Bachelor of Social Work degree
- Experience in working with Geriatrics
- Familiarity with the multi-disciplinary team concept
- Skill in teamwork and ability to work effectively in a multi-disciplinary setting
- Ability to initiate and direct own departmental activity
- Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
- Ability to work in a position of trust with vulnerable adults (residents)
