- Hospital Social Worker providing psychosocial assessments, crisis intervention, and discharge planning across medicine, surgery, emergency, and oncology. Linking patients with community resources.
Responsibilities
- Report to the Patient Care Manager of Social Work
- Provide social work services to a diverse patient population
- Provide services across general medicine, surgery, emergency, and oncology programs
- Perform psychosocial assessments and support discharge planning
- Link patients to community resources
- Provide crisis intervention and supportive counseling as applicable
- Collaborate with patients, families, co-workers, and interdisciplinary teams
- Follow SHN’s Code of Conduct and health and safety policies
- Use protective equipment and identify and report hazards
- Foster an ethical, inclusive, healthy, and positive work environment
Requirements
- MSW degree and registration with the Ontario College of Social Workers and Social Service Workers
- Experience in a hospital setting
- Excellent crisis intervention skills
- Strong focus on psychosocial assessments and discharge planning
- Excellent assessment skills and familiarity with community services
- Ability and interest in linking patients to community resources
- Flexibility and ability to work as a strong team player
- Excellent verbal and written interpersonal and communication skills
- Good judgement when relating to patients, families, co-workers and interdisciplinary teams
- Values consistent with SHN’s Code of Conduct
- Commitment to health and safety policies and legislation
- Ability to utilize protective equipment and identify and report hazards
- Ethical conduct and appreciation of patient, staff and community diversity
- Excellent work performance and attendance record
- Ability to speak another language is an asset
