About the role

Social Worker providing services at Scarborough Health Network's hospitals. Responsible for diverse patient population across medical programs and discharges.

Responsibilities

Reporting to the Patient Care Manager of Social Work, you will be responsible for a diverse patient population.
Social workers provide service across many programs including general medicine, surgery, emergency, and oncology.

MSW degree and registered with the Ontario College of Social Workers and Social Service Workers
Experience in a hospital setting
Excellent crisis intervention skills
Strong focus on psychosocial assessments and discharge planning
Excellent assessment skills and familiarity with community services
Strong ability and interest in linking patients to community resources
Must be flexible and a strong team player
Demonstrates excellent interpersonal and communication skills (verbal & written) and good judgement when relating to patients, families, co-workers and interdisciplinary teams
Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
Utilize protective equipment provided and identify and reports hazards
Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
