Responsibilities
- Reporting to the Patient Care Manager of Social Work, you will be responsible for a diverse patient population.
- Social workers provide service across many programs including general medicine, surgery, emergency, and oncology.
Requirements
- MSW degree and registered with the Ontario College of Social Workers and Social Service Workers
- Experience in a hospital setting
- Excellent crisis intervention skills
- Strong focus on psychosocial assessments and discharge planning
- Excellent assessment skills and familiarity with community services
- Strong ability and interest in linking patients to community resources
- Must be flexible and a strong team player
- Demonstrates excellent interpersonal and communication skills (verbal & written) and good judgement when relating to patients, families, co-workers and interdisciplinary teams
- Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation
- Utilize protective equipment provided and identify and reports hazards
- Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
