About the role
- Bilingual Nurse Case Manager providing support to patients and healthcare providers in Canada. Collaborating with healthcare teams to ensure effective treatment management and documentation.
Responsibilities
- Provide medical support to patients and healthcare providers
- Act as a liaison between patients, families and healthcare providers
- Document patient information and interactions in the electronic patient record
- Collaborate with the reimbursement specialist and the physician to secure funding
Requirements
- Bachelor's degree or licensed nurse (IA)
- Minimum of two (2) to five (5) years of experience
- Bilingual (French and English) required
- Experience in hematology and/or nephrology is an asset
