About the role

  • Mental Health Counsellor providing assessment, treatment, and chronic disease support within Ontario’s collaborative primary-care network. Delivering evidence-based care virtually and in person.

Responsibilities

  • Provide assessment, ongoing treatment and management of biopsychosocial aspects of health and well-being for identified patients
  • Participate in the development, implementation, monitoring and evaluation of programs for the TVFHT patient population
  • Collaborate with an Inter-professional Healthcare Provider team
  • Assist with health promotion, disease prevention, and chronic disease management across patients’ lifespans
  • Provide virtual and in-person care
  • Monitor and evaluate service delivery for individual clients, communities, and populations using mental health and addictions measurement tools and technology
  • Deliver efficient primary care, including sharing information and teaching
  • Develop and facilitate groups
  • Work independently and collaboratively as a member of a multidisciplinary team

Requirements

  • Graduate of an accredited University
  • Registered and in good standing with a regulated health professional College, including the College of Nurses of Ontario, Ontario College of Social Workers and Social Service Workers, or College of Occupational Therapists of Ontario
  • Minimum of five years clinical work experience
  • Extensive mental health clinical training, generally acquired through post graduate studies
  • Knowledge and experience in mental health treatment and management, including evidence-based treatments such as CBT
  • Advanced mental health knowledge and experience working with adults, children, youth, and families, acquired through a minimum of 3–5 years supervised clinical practice in this area
  • Understanding of addictions care and navigation
  • Postgraduate studies such as MSW, OT, RN, BScN, CPMHN(C) or equivalent, and membership in a professional college
  • Minimum of 3 years’ experience assessing and managing mental health presentations commonly seen in primary care
  • Experience delivering efficient primary care, including sharing information and teaching
  • Experience developing and facilitating groups
  • Knowledge and proficiency in evidence-based social work methods and practices in primary care settings, with emphasis on chronic disease management, health promotion, and risk reduction
  • Ability to monitor and evaluate service delivery using mental health and addictions measurement tools and technology
  • Knowledge of relevant health care legislation and community mental health resources
  • Understanding of social determinants of health and their impact on health and well-being
  • Leadership, organization, research, evaluation, time management, and interpersonal skills
  • Ability to work independently and collaboratively as a member of a multidisciplinary team
  • Desire and ability to update knowledge and skills through technology-based opportunities, courses, workshops, and conferences
  • Active engagement in the performance management program
  • Comfort providing in-person and virtual care
  • Experience using an EMR, Microsoft Office, and measurement-based care platforms
  • Ability to be open and non-judgmental
  • Effective written and verbal interpersonal and communication skills
  • Successful candidate must undergo a criminal background check and medical screening
  • Must have a Social Insurance Number and, in some cases, a valid work permit
