About the role

  • Veteran Care Coordinator supporting Canadian veterans with medical cannabis coverage, appointments, and documentation. Coordinating care with healthcare teams, Veterans Affairs Canada, and Medavie Blue Cross.

Responsibilities

  • Support veterans through appointment scheduling, triage, education, and ongoing assistance with Veterans Affairs Canada coverage
  • Facilitate counselling services and Veterans Affairs Canada assessments as needed
  • Manage calls and emails directed to the Veteran phone line or email account
  • Coordinate patient coverage by submitting registrations and supporting documents to Licensed Producers, verifying interim coverage eligibility, submitting documents to Medavie Blue Cross, and arranging vaporizer coverage
  • Handle inquiries from Medavie Blue Cross and Veterans Affairs Canada regarding patient information, invoice submissions, and payment issues
  • Manage Licensed Producer changes and coverage splits, following up with Medavie Blue Cross to prevent disruptions
  • Schedule and prepare appointments, ensuring relevant documentation is on file
  • Provide patient support, process documents, and schedule follow-up appointments
  • Conduct proactive follow-ups with veteran patients, monitor treatment progress, address medication access issues, and track outcomes in the EMR and tracking documents
  • Monitor prescription, exceptional coverage, and vaporizer coverage expiration dates
  • Collaborate with clinic teams to expedite patient care and enhance the patient experience
  • Maintain and update patient trackers with patient numbers, order confirmations, prescribed dosages, expiration dates, and vaporizer coverage
  • Perform other duties as assigned

Requirements

  • Exceptional attention to detail and strong organizational skills
  • Knowledge of Licensed Producers and available medical cannabis products
  • Ability to educate patients on cannabis strains, consumption methods, and titration
  • Strong problem-solving skills in a fast-paced environment
  • Excellent verbal and written communication skills
  • Bilingualism preferred but not mandatory
  • Ability to adapt to changing priorities
  • Familiarity with medical terminology and pharmacology is an asset
  • Experience with EMR systems, Microsoft 365, and QuickBooks
  • Compassionate, patient-focused approach with strong interpersonal skills
  • Ability to work independently and collaboratively as part of a team
