About the role

  • Bilingual Nurse Case Manager supporting patients, providers, and medication access for Cencora’s Canadian pharmaceutical programs. Coordinating case management, therapy funding, adherence, and adverse event reporting.

Responsibilities

  • Provide support to patients and healthcare providers with information about Patient Support services, diseases, and therapies
  • Assist patients and caregivers with assessments related to Patient Support services
  • Liaise between patients, families, and healthcare providers regarding medication access
  • Address inquiries about program services, disease states, and products
  • Provide ongoing support to promote patient adherence
  • Refer patients to healthcare providers for clinical questions beyond program materials
  • Document patient information and interactions in electronic patient records
  • Conduct ongoing file reviews and updates throughout the patient journey
  • Follow case-management protocols, including data collection, follow-up, therapy disruption/discontinuation monitoring, adverse event reporting, and liaison with primary healthcare providers
  • Collaborate with reimbursement specialists and physicians to secure therapy funding and explore alternatives
  • Coordinate and track patient services according to KPIs to expedite therapy access
  • Participate in specialized therapy training and pharmaceutical meetings
  • Attend patient meetings and conferences and work with patient associations and advocacy groups as needed
  • Coordinate with healthcare providers, pharmacies, and infusion/injection clinics on treatment logistics and issue resolution
  • Maintain communication with the manager, team, and client regarding patient cases
  • Assist with training new Nurse Case Managers
  • Report adverse events and severe adverse events according to program procedures
  • Perform other duties as assigned and participate in standby/on-call coverage when required

Requirements

  • A Diploma in health care or a related field; a bachelor’s degree is preferred
  • Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment is required
  • Minimum of two (2) to five (5) years in the healthcare, pharmaceutical, insurance or related fields
  • Healthcare and case management background in medical/hospital settings, insurance or bio-pharmaceutical industry preferred
  • Fluency in French and/or English may be required depending on program and geographic location
  • Experience with patient support, case management, medication access, reimbursement, adverse event reporting, and electronic patient records
  • Ability to work autonomously and as part of a team
  • Strong analytical and problem-solving skills
  • Ability to work effectively across functions and within a small team
  • Ability to consistently deliver high-quality professional work in a dynamic environment
  • Experience training new Nurse Case Managers and providing feedback on protocols, databases, processes, and interdepartmental processes
  • May be required to be on standby/on call
