About the role

Responsibilities
- Act as a resource person for the interprofessional team
- Follow up on abnormal results and coordinate the necessary procedures
- Monitor tests prescribed in GMF-R clinics and requests from the clinical reception desk
- Provide patients with education, self-management support, and required follow-up care
- Direct patients to appropriate services and coordinate with external partners
- Participate in the development of clinical tools and quality-of-care monitoring
- Strengthen collaborative relationships with healthcare network partners
- Work under the supervision of the nursing leadership team
Requirements
- Diploma of College Studies (DEC) in Nursing (required)
- 3 years of nursing experience (preferred)
- Strong interest in patient relationships and interdisciplinary collaboration
