About the role
- Registered Nurse managing remote care for pediatric and palliative patients at VON. Directing PSWs, utilizing electronic documentation technology, and ensuring adherence to care protocols.
Responsibilities
- Manage pediatric, palliative and/or acute medically complex clients with COPD or CHF
- Direct Personal Support Workers (PSWs) to provide proper care
- Provide remote assessment and management of care services to clients
- Maintain accurate client clinical records
- Collaborate with multi-disciplinary health care team
Requirements
- Registered and in good standing with the College of Nurses of Ontario
- CNA Certificate in Palliative Care or Fundamentals of Palliative Care and Comprehensive Advanced Palliative Care Education (CAPCE)
- Extensive hands-on experience in Pediatric Care
- Recent experience with COPD and CHF patients required
- Digital and electronic health experience, EMR and mobile technology
- Valid CPR C / First Aid required
- Access to a personal vehicle for business use and possess a current driver’s license and proof of vehicle insurance
- Current and original copy of a satisfactory Criminal Records Check is required
