About the role

  • Rehabilitation Service Specialist coordinating medical, psycho-social, and vocational support for Canadian Armed Forces veterans. Developing personalized plans and reducing barriers to successful community re-establishment.

Responsibilities

  • Determine required rehabilitation assessments to assess Participants’ capacity for improvement and clarify rehabilitation needs
  • Coordinate rehabilitation assessments and services as required
  • Ensure services and plans are needs-based, goal-oriented, aligned with program mandates, safe, cost-effective, and high quality
  • Develop thorough Rehabilitation Plans in consultation with Participants, families/caregivers, Rehabilitation Service Professionals, and Veterans Affairs Canada Case Managers
  • Act as the service integrator across medical, psycho-social, and vocational rehabilitation services
  • Empower Participants to problem-solve using community-based interventions and resources
  • Identify barriers or risk factors contributing to suboptimal outcomes
  • Proactively develop and implement strategies to improve outcomes
  • Provide rehabilitation services to veterans of the Canadian Armed Forces and their families
  • Travel minimally across the region as required

Requirements

  • Bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related field
  • One of: a health professional designation; Certified Vocational Rehabilitation Professional (CVRP or CVRP-F); or, in Québec, registration as a Career Counsellor with the Ordre des conseillers et conseillères d'orientation du Québec
  • Minimum 3 years of experience directly providing medical, psycho-social, or vocational rehabilitation services, or a combination thereof
  • Experience working with persons living with mental or physical health problems that create barriers in life
  • Employment conditional on obtaining federal security clearance – Reliability Status
  • Ability to travel across the region as required
  • Bilingual French/English preferred
  • Experience working with employers, community resources, and training institutions preferred
  • Current and former members of the Canadian Armed Forces are encouraged to apply
