- Care Navigator guiding older adults and caregivers through Sailor Health’s mental-health intake process. Coordinating therapist matching and providing compassionate patient support remotely from Canada.
Responsibilities
- Serve as the first voice patients or their families hear when contacting Sailor Health
- Guide older adults and caregivers through the intake process
- Provide patient support, care coordination, and emotional guidance
- Help patients navigate entry into care
- Match patients with the right therapist
- Support patients throughout the intake and care-navigation journey
- Help patients and families feel supported, understood, and confident
Requirements
- Based in Canada
- Strong empathy and clear communication skills
- Active listening and ability to make people feel at ease during difficult conversations
- Highly organized and detail-oriented
- Ability to manage multiple patients at different stages
- Comfortable learning Google Workspace, EHR platforms such as Healthie, and CRMs such as HubSpot
- Ability to quickly learn new software
- Interest in improving care for older adults and mental healthcare access
- Experience in healthcare, customer service, or administrative support is welcome but not required
- Willingness to work Eastern Time Zone hours
- Willingness to work one of the listed shifts between 9 a.m. and 9 p.m. ET
