About the role
- Registered Practical Nurse providing scheduled in-home care for ParaMed, Canada’s home healthcare provider. Assessing patients, monitoring care plans, and maintaining clinical records.
Responsibilities
- Conduct scheduled visits to care for patients in various settings
- Care for patients of all ages with differing complexity of care needs
- Assess patients’ individualized needs using appropriate assessment tools and techniques
- Execute and continuously monitor care plans
- Engage patients collaboratively by building rapport and trust
- Maintain and update professional patient records related to patient encounters and plans
- Provide care that helps patients remain comfortable in their homes
- Provide required on-call coverage, including after hours
Requirements
- Registration with the CNO as a Registered Nurse / RPN in good standing
- Valid driver’s license and access to a vehicle is required
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Passion for making a difference in your community
- Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
