About the role
- Palliative Registered Nurse providing in-home care across Kitchener and Cambridge for ParaMed, Canada’s home healthcare provider. Assessing patients, monitoring care plans, and supporting comfort at home.
Responsibilities
- Provide scheduled palliative nursing visits to patients in various settings
- Care for patients of all ages with differing complexity of care needs
- Help patients remain comfortable in their homes for as long as possible
- Use appropriate assessment tools and techniques to assess individualized patient needs
- Execute and continuously monitor care plans
- Engage patients using a collaborative approach by building rapport and trust
- Maintain and update professional patient records related to patient encounters and plans
- Provide required on-call coverage, including after hours
- Travel anywhere within Kitchener and Cambridge
Requirements
- Registration with the CNO as a Registered Nurse in good standing
- Valid driver’s license and access to a vehicle is required
- Palliative care experience is mandatory
- Post-graduate education in Palliative care is required
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area

- Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
