About the role
- Wound Care Nurse delivering advanced wound, ostomy, and continence care through ParaMed’s home healthcare services. Educating nursing teams, patients, and families across North York and Vaughan.
Responsibilities
- Provide direct practice to patients requiring wound, ostomy and continence assessment, care and management
- Provide education to ParaMed’s nursing health care team, patients, and families
- Provide consultation and clinical support to ParaMed’s nursing health care team, directly or remotely, including recommendations for specialty products and nursing care
- Participate in wound, ostomy, and continence clinical leadership opportunities
- Collaborate with internal and external stakeholders
- Preceptor ParaMed nurses as required
- Follow wound, ostomy, and continence evidence-based clinical best practices and ParaMed policies and procedures
Requirements
- Nurse Specialized in Wound Ostomy & Continence (NSWOC) OR International Interprofessional Wound Care Course (IIWCC)
- Skin Wellness Associate Nurse (SWAN)
- Other wound care certifications may be considered
- Registration with the CNO as a Registered Nurse in good standing
- Minimum of 1 year experience as a practicing Registered Nurse, ideally in a community setting and/or providing wound/ostomy/continence care to patients (direct care/consultation)
