About the role
- Palliative RPN delivering community-based home health care across Kitchener and Cambridge. Providing safe nursing, assessments, care-plan monitoring, and patient support.
Responsibilities
- Conduct scheduled community visits to care for patients in various settings
- Care for patients of all ages with differing complexity of care needs
- Provide ethical and safe nursing care within scope of practice
- Assess patients’ individual needs using appropriate assessment tools and techniques
- Execute, continuously monitor, and adjust care plans as required
- Build rapport and trust with patients using a collaborative approach
- Maintain and update professional patient records related to encounters and care plans
- Provide required on-call coverage, including after hours
Requirements
- Registration with the CNO as a Registered Practical Nurse in good standing
- Palliative care experience is mandatory
- Post-graduate education in Palliative care is required
- Valid driver’s license and access to a vehicle is required
- Experience in delivering community care is an asset
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area
- Experience working with the vulnerable population

- Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
