About the role
- Pediatric RN providing scheduled school-based care for complex pediatric patients through ParaMed, Canada’s home health care provider. Assessing needs, monitoring care plans, and maintaining patient records.
Responsibilities
- Care for pediatric patients in a school setting during scheduled shifts
- Work with pediatric patients with differing complexity of care needs
- Help patients remain comfortable
- Assess patients’ individualized needs using appropriate assessment tools and techniques
- Execute and continuously monitor care plans
- Engage patients using a collaborative approach by building rapport and trust
- Maintain and update professional patient records related to patient encounters and plans
- Provide required on-call coverage, including after hours, to ensure patient safety and meet patient care needs
Requirements
- Registration with the CNO as a Nurse in good standing
- Valid driver’s license and access to a vehicle is required
- Experience in Pediatric care is mandatory
- Experience in delivering community care is an asset
- Clinical skills in gastric feed, tracheostomy, and ventilation care strongly preferred
- Reliability and flexibility
- Willingness to travel to and from patient residences within a designated area

- Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs
