About the role
- Bilingual Nurse Case Manager at McKesson coordinating patient care and education for specialty therapies. Supporting adherence through direct clinic and home interactions, ensuring smooth access to treatment.
Responsibilities
- Act as the primary contact for prescribers and clinic teams to enroll patients and coordinate services.
- Provide patient education (disease state, medication, side‑effect management) and deliver in‑home, in‑clinic, or virtual injection training based on patient preference.
- Support adherence through scheduled follow‑ups; share timely clinical updates with physicians/clinic staff.
- Complete concise, accurate documentation; conduct chart reviews as needed for reimbursement navigation.
- Collaborate with McKesson pharmacy colleagues, manufacturer partners, and clinic teams to remove barriers to care and escalate issues beyond program scope when required.
Requirements
- Bilingual: Fluent in French and English (spoken and written).
- Nursing license in Ontario or Quebec (or both).
- RN strongly preferred; RPN/LPN and NP considered.
- 2+ years of relevant experience (or an equivalent combination).
