About the role

  • Field Case Manager contributing to patient management and reimbursement processes for healthcare solutions company. Delivering support while ensuring compliance and quality service measures throughout patient interactions.

Responsibilities

  • Practice management
  • Customize program elements to meet the needs of designated prescribing physicians
  • Ensure compliance with nursing standards set by the nursing regulatory authority
  • Review patient status and assist the prescribing physician with completing patient enrollment
  • Proactively evaluate programs and make recommendations to improve outcomes and service quality
  • Welcome each patient to the program and record required patient information
  • Provide treatment guidelines to patients and physicians
  • Ensure patient services are coordinated and monitored according to key performance indicators (KPIs)
  • Coordinate private and public insurance coverage activities directly with insurers
  • Report adverse events/serious adverse events in accordance with the program's standard operating procedures
  • Complete all relevant reports (timesheets, expenses, mileage, etc.) according to established deadlines.

Requirements

  • Registered Nurse (RN) or Registered Practical Nurse (RPN); registration with the College of Nurses of Ontario; case management experience preferred
  • Minimum of three (3) years of clinical experience is an asset
  • Available to travel within an assigned territory; occasional overnight travel may be required
  • BCLS (Basic Cardiac Life Support) certification may be required depending on the program's therapeutic area
  • Valid driver's license, current auto insurance, and access to reliable transportation for regional travel
  • Experience and training across various therapeutic areas is an asset
  • Knowledge of public and private reimbursement structures, systems, and processes
  • Fluency in French and English is required.
