Responsibilities
Supervise the operations of the division's Supportive Housing (SH) and Homemakers and Nurses Services (HMNS) program
Implements detailed plans and recommends policies/procedures regarding program specific requirements
Supervises, motivates and trains assigned staff, ensuring effective teamwork and high standards of work quality
Monitors and evaluates staff performance and approves salary increments
Investigates concerns and complaints brought forward by clients and families
Engages divisional resources; consults with Legal, Ombudsman, Human Rights offices
Plans, develops and directs the effective delivery of services to clients
Manages and maintains collaborative partnerships with contracted agencies and community organizations
Ensures accurate maintenance of clients' healthcare records
Requirements
Post-secondary education with a degree in Nursing and/or another professional discipline pertinent to the job function
Considerable experience planning, directing and co-ordinating community-based programs
Considerable experience motivating, leading and supervising regulated and unregulated staff in a unionized community setting
Experience managing complex client situations involving high risk seniors and/or vulnerable populations in a community setting
Experience working collaboratively with external partners, such as community groups, community service agencies, hospitals, staff, clients and families
Highly developed interpersonal, communication (written and verbal), negotiation, conflict resolution skills
Ability to develop, deliver and evaluate programs with a quality improvement focus
Ability to navigate computer systems, software and applications such as Microsoft Word and Excel
Sensitivity and understanding of the diversity of knowledge, languages, literacy levels, culture and ethno-racial background in communities throughout Toronto
