Bilingual Nurse Case Manager in McKesson's Patient Support Programs. Coordinating patient education and adherence support across the treatment journey in Canada.
Responsibilities
Act as the primary point of contact for prescribers and clinical teams to enroll patients and coordinate services.
Provide patient education (disease, medications, management of side effects) and deliver injection training for at-home administration, in-clinic, or virtually, according to patient preference.
Support treatment adherence through scheduled follow-ups and promptly communicate clinical updates to physicians and clinical staff.
Perform concise and accurate documentation, and conduct chart reviews as needed to support reimbursement.
Collaborate with McKesson pharmacy colleagues, manufacturer partners, and clinical teams to remove barriers to care and escalate issues beyond the program’s scope when necessary.
Requirements
Bilingualism: Proficiency in French and English, both spoken and written.
Nursing license in Ontario or Quebec (or both).
Registered Nurse (RN) designation is strongly preferred; Registered Practical Nurse/Licensed Practical Nurse (RPN/LPN) and Nurse Practitioner (NP) applicants will be considered.
Minimum of 2 years of relevant experience (or an equivalent combination of education and experience).
